By registering for programs with Impact School of Dance Performance,

I acknowledge and accept these risks.

I certify that my child is physically able to participate in dance activities and agree to assume full responsibility for any injuries or accidents that may occur during participation in classes, rehearsals, or related activities.

I hereby release and hold harmless Impact School of Dance Performance, its instructors and any facility used for classes from any and all liability, claims, or damages arising from participation in the program.
